It depends on your bank. My bank - Charles Schwab - does not charge any fees and offers commercial exchange rates for foreign ATM withdrawals. This is the best way to get cash. Not all banks accept US ATM cards, but Citi and Deutsche Bank ATMs do. So I just go to Deutsche Bank.
