Drives: 2013 BMW M5 (manual)

Mysterious Hole

Good evening. Just learning forum life. Need some help/thoughts on this problem.

Background:
-recently replaced two water pumps and had the cooling lines repaired over the top of the block.
-90,000 miles on the M5 (6 spd)
-Check engine (yellow) light came on – OBDII said System Too Lean Bank 1 – P0171
-Low and behold a hole was observed in the front of the heat exchanger box – projectile came out and nicked the bar in front of it.

Obvious concerns of something coming loose from inside the engine? Did something get dropped during one of the repairs? What other damage might have occurred?
Dealership is investigating and supposedly reaching out, I believe to BMW NA or whomever could make a decision to help pay for some of the costs due to the rarity of the situation.
