Was able to speak to some people elsewhere and was able to get a video to them. They suspect it may be the cam follower / lifter for the high pressure fuel pump. $40 OEM part and a $5 gasket. Going to give it a try next week just for peace of mind as I'd hate to hurt a cam and we'll go from there. Comes and goes for me as well.
