Yeah, a mismatch in height would definitely make things more difficult. I really can't think of any other position though. I've tried every single degree of angle I think and that's the only one I've come to find is semi-comfortable.

That being said, I've found that if she curls up as well (away from you, such that you are wrapping a little more of your body around her), you'll be less at the knees and a few more inches closer to glory.
