Fault code 118111 mixture too rich bank 2 help.

Had the code a while back with a rail pressure plausibility fault. Cleared and I have since flashed to stock factory tune. After almost 1,000 miles 118111 finally reappeared. No other codes this time. No drivetrain errors or limp mode. Idles perfect, drives perfect.

Hooked up ISTA, and started troubleshooting. Injector test passed. During the O2 test, bank 1 heated up almost instantly, however, bank 2 never heated up. Ran the car for several min at 2k RPMs. So I swapped precat O2s left to right and the problem did not follow.

Mods are MSR intakes, ESS high flow catted downpipes, and Akrapovic exhaust. Originally had the ESS tune with low boost.

There aren't any visible leaks or anything obvious that I can tell. Is it possible I have a leaky injector? Any thoughts?

I won't rule it out, but it's unlikely since the intakes have been installed on the car for years with no issues till now.

i have msr intake and same error code there, i cleared the code and it gone, no more, i must read the codes again because no error or drivetrain there or anything .. if you found whats the problem let me know

Ended up being leaky injectors. Replaced myself about 1000 miles ago and all is well so far.
